3. California customers please add sales tax of 8.25% of the subtotal
4. US shipping is $3 first piece and 50 cents each additional piece. Shipping outside USA is $6 for the first CD and $2 for each additional CD.
5. Mail the form or fax it to the company address with payment or payment information. Visa, MC and American Express are welcomed. International orders must be paid by credit card or money order drawn in US dollars.
