Complete the Order Form:
Using the Order Form below, fill in the contact information and select the items and quantities you want to order. Once you have selected the items you want, click the "Next" button. This will display an "Order Confirmation" page with your completed order and the total cost due. Print the page for your records. Or you can print out a Blank Order Form and mail it to us.

Placing an order by Mail:
Once you have completed the Order Form or Blank Order Form, if you wish to order by mail, make an extra copy of the "Blank Order Form" or the "Order Confirmation" page and mail it with a check or money order to our Post Office Box. Your order will be processed and returned usually within 7-10 working days. If you used this page to place an order, click the "Place Order" button on the Order Confirmation page to send us your order information.

Placing an order Online:
You can also order using your credit card online. We currently can not directly process credit cards so we offer two ways to pay with your card. You can pay using PayPal.com or Kagi.com.

When you click the "Next" button on this page, your order information will display in an "Order Confirmation" page. Print out the Order Confirmation page for your records and then click the "Place Order" button that appears on the Order Confirmation page. This will send your order information to us and display your payment options.

You will need to have the Amount Due for your order (from the "Order Confirmation" page) available to enter into either PayPal.com or Kagi.com.

PayPal.com is a secure, easy way to transfer money to anyone with an email account. To pay through PayPal, you will need to register if you are not a PayPal member. For a limited time PayPal will give you a $5 credit on your account when you join. Joining PayPal and sending payments through PayPal is free.

Kagi.com is a credit card payment service. To pay through Kagi, you go directly to our Pocket Pediatrics Order page. When entering your Amount Due on the Kagi.com Order page, you must round the Amount Due off to the next dollar for your order to be processed correctly.

Both services offer secure orders using your credit card. PayPal gives you a $5 credit but requires you to join their service. You can use PayPal to pay anyone with an email address any amount through your credit card. Kagi does not require you to join but can only be used to pay for our products.

Once your credit card information is processed by either service, they will forward us a notice and we will fill the order. This is usually within 48 to 72 hours so you will get your order more quickly if you order with a credit card.
